We achieve this by increasing the efficiency of the discharge pathway for individuals who are ready to step down from an acute setting. Working alongside your discharge management team, we liaise with key stakeholders, including family members and the MDT, to support an individual’s journey to sustained independence.
We support ICS’s, NHS trusts, and local authorities to review existing section 117 care plans and ensure they:
Free up acute bed space for those most in need of support
Provide your team with additional capacity to improve patient flow
Safely return people to their home or, most appropriate setting
Deliver expert support to individual with complex comorbid physical and mental health needs
Reduce out-of-area placements and re-admissions
