NHS Continuing Healthcare Assessment and Case Managers

Location: Sutton Coldfield with national travel

Salary: £32k – £40k depending on experience  

Hours: Flexible hours, full time or part time

About the role

This is a clinical role (Nursing, Physiotherapy, Occupational Therapy, or Social Work) within the Clinical Services team, accountable for delivery of Continuing Healthcare assessment and case management quality standards in line with the National Framework for Continuing Healthcare and NHS Funded Nursing Care (revised 2018).

This role should attract both experienced clinical CHC practitioners and also those persons working within aligned processes; for example PIP assessment, BIA experience and Mental Health practitioners. CHS welcome clinical applications from both NHS and Independent Sectors.

You will work as part of the clinical services team to ensure that continual quality improvement remains integral to the successful delivery of contracts and service specifications.

You will be based at our offices in Sutton Coldfield, with a requirement for regular travel throughout the country.

The work of the Clinical Services Team includes NHS Continuing Healthcare, Funded Nursing Care, Mental Health, Neuro Rehabilitation and Deprivation of Liberties (CCG). It also supplies Caretrack and Broadcare patient management database systems.

You will work in a fast paced, inspiring environment, with key stakeholders including senior NHS and social care managers to develop existing relationships. You will also have chance to work in the community, in local care homes and hospitals with the opportunity to build great relationships with your patients, customers and your team.

What sort of person are we looking for?

We are looking to recruit experienced clinical practitioners, with excellent knowledge (or potential to develop) of NHS continuing healthcare operations at both national and local level. As a qualified clinician, you must be registered with a professional body and be able to demonstrate a high level of professional expertise.
