We’ve been working with health and social care for over 19 years and as it’s integrated care week, we’re reflecting on our partnership work in Bath and North East Somerset. We work with the Council as well as the CCG to support patients out of hospital and into the right care. Our work with these partners has demonstrated how integrated work can drive reduced DTOC figures from 114 days to 49 days, keep nursing home rates stable and ensure patients and their families feel better supported and more informed around their options. 95% rated our service as excellent or good.
