COVID-19 Reset and Recovery
In response to the current pandemic and following the COVID-19 Hospital Discharge Service Guidance and the communication which has been issued to NHS Providers and Commissioners, CHS Healthcare is able to support health and care systems with the safe and rapid discharge of the thousands patients from acute trusts to help free up capacity for the care of Covid patients.
We are currently located in more than 130 Hospitals, supporting their Covid efforts by discharging hundreds of patients each day, and we are working with over 160 Clinical Commissioning Groups, to enable a coordinated system wide approach. We are also experienced in setting up and implementing all D2A pathways and the management of large interim care bed bases. We have a bespoke care coordination tracking system to enable ongoing oversight of patients in the community. We can conduct safe and well assessments on large populations of patients and also follow up on all funding decisions in the short and longer term.
